Care Plans

A care plan starts on your arrival.

During your first month at Chelston we will develop your care plan. Alongside your family or representatives, you are invited to participate in its development.

You or your representative may have unrestricted access to your care plan by arrangement with the registered manager.

Care Plan Review

After this initial period, your care plan will be reviewed regularly to ensure that it is individualised to you and focused around your changing needs.

Where possible and practical, family and friends are encouraged to participate in your daily routine and where appropriate, they are invited to formal reviews. You or your relatives, friends and representatives are always welcome to discuss any concerns with the nurse manager or the nurse in charge.

Your care plan is reviewed regularly:

  • Firstly, we look at and adjust your care plan at the end of the four-week settling-in period.
  • The care plan is then reviewed daily – on a shift-to-shift basis. At shift changeover, the outgoing staff brief incoming staff on your care notes. Your health needs, any instructions from allied health professionals, as well as your responses and activity patterns are discussed and changes to the care plan may be proposed at this point.
  • Formal reviews are held with care staff on a regular basis. This usually takes place monthly unless your needs change and a review is required sooner.

All amendments to your care plan are considered according to the guidance on the Mental Capacity Act and a Best Interest Assessment and require the authorisation of the Registered Manager or Clinical Lead and your allocated nurse. Certain amendments may require the authorisation of your GP too. All amendments to a care plan are recorded in full.

Medication

Medication is managed by the nurses and dispensed and ordered under the instructions of the GP. If it is safe to do so, residents are welcome to administer their own medication. In such cases, a nurse’s help and advice is available. Residents may request to see a GP in private and the Chelston Park will assist in arranging this.

Nourish Digital Care Planning Software

We use a state-of-the-art care planning software called Nourish. Nourish uses dedicated handsets where staff input care interactions onto a timeline. This on-the-feet tracking of care interactions frees our staff to spend more time with clients than traditional care planning methods allow. Nourish also generates useful charts that our nurses use to monitor care.

Nourish provides a portal that allows health care professionals, social services and—in some circumstances—family members to view and review the care delivered.
